  Add name of incapacitated person’s representative 
                     This is for the bondholder declared by the court order to be an incapacitated person. The curator who represents the incapacitated person must submit the application/change form requesting to add his or her name in the bond record as a curator.

1) Registration and Records Application / Change Form
2) Curator’s specimen signature card (must be signed in the presence of the BOT or selling-agent bank)
2. Required documents
1) Certificate of bond or debt security
2) A copy of court order declaring a bondholder to be incapacitated person and appointing curator
3) A copy of the curator’s identification card
4) A copy of bank account passbook in the case of name change or bank account change
**All documents certified as true copies**
3. Fee
20 Baht per security certificate
4. Contact procedure
1) Contact in person at the BOT, or
2) Contact selling-agent bank
5. Additional note
1) In the case where the bondholder declared by court order to be a semi-incapacitated person, the guardian must submit an application for additional specimen signature card.
2) Whenever the semi-incapacitated person takes legal action, it must receive permission from the guardian.
