5. Additional note

1) In the case of estate administration, if the administrator requests to transfer ownership or receive principal of the debt securities, it is required that an Application Form for Ownership Transfer or an Application Form for Receiving Principal of Debt Securities, as the case may be, be attached.

2) The administrator must present a copy of court’s order certified by a judicial officer.
